resolveVersions({ refresh: true }) performs outbound HTTPS to the npm registry; it needs host approval on first use and falls back to pins on failure.

Edge cases

• Unknown host in planSetup does not throw — it plans with a warning to confirm via npx skills list -g.
• Unknown template in renderTemplate and unknown kind in validatePaste throw with the known-value list.
• No local side effects anywhere: no PATH checks, no file writes, no installs. The caller acts on outputs.
